We are looking for a passionate, dedicated individual to serve as our Admissions Coordinator. The Admissions Coordinator is an integral part of the sales and marketing team at Carillon. As the new Admissions Coordinator, you will help us to drive census by qualifying potential residents for admission to Carillon, providing an extraordinary first impression to new and prospective residents, and developing positive relationships with Carillon's stakeholders. ESSENTIAL JOB DUTIES:

  • Responsible for meeting the heath center's sales and occupancy levels in accordance with the strategic marketing plan. Results will be achieved through coordinating with outreach liaisons, Independent Living needs, health center tours, follow-up telephone calls and appointments to keep the professionals informed.
  • Develops positive relationships with the discharge planners, physicians and families, marketing Carillon House programs and services.
  • Assesses potential residents/guests, reviewing their medical records, and determines whether they should be admitted for care in collaboration with physicians, nurses, therapists, social workers, case managers and other interdisciplinary team members. Obtains prior authorizations as required.
  • Reviews all admission related paperwork with the resident and or/responsible party including financial data, signatures, pertinent social information and medical information, attending physician information, Medicare Part A coverage and non-coverage procedures, and other pertinent and required documentation.
  • Coordinate pre-admission process to include clinical communication of needs, pharmacy needs, financial information, durable medical equipment needs, etc.
  • Manages Salesforce data entry daily.
  • Works in conjunction with LCS to create and disseminate relevant marketing collateral.
  • Informs the Interdisciplinary Team of on-going daily census activity such as admissions, discharges, deaths, room transfers, changes in payment methods, and temporary and permanent apartment status.
  • Welcomes newly admitted residents to the community upon admission or prior.
  • Maintains complete confidentiality regarding admission information and resident records.
  • Assists in coordinating admissions/transfers/discharges with other departments.
  • Other duties as assigned by the Administrator

QUALIFICATIONS:

  • Good working knowledge of Medicare Part A/HMOs/Insurances, certification procedures, and medical terminology.
  • 1+ year of long-term care experience is preferable.
  • Able to work flexible hours.
  • Familiar with federal and state regulations and requirements for long-term care.
  • Must possess excellent written, oral and interpersonal skills, as well as the ability to juggle multiple priorities.
  • Case management experiences a plus.
  • Familiar with and adhere to guidelines related to the Fair Housing Act (FHA) and the Americans with Disabilities Act (ADA) regulations in all aspects of the job including sales presentations and marketing materials.
  • Must be an effective presenter with both large and small groups.
  • Must have the ability to perform simple arithmetic.
  • Must be able to generate a warm, friendly, and caring manner on first impression.
  • Be able to work cooperatively with other healthcare professionals and other departments within the system.

PERSONAL AND PUBLIC CONTACT

The Health Services Coordinator will have personal contact with all levels of Carillon staff, residents, other healthcare professional, vendors, community agencies, and LifeCare staff.

PHYSICAL/MENTAL CONDITIONS

Normally works in well-lighted, comfortable surroundings. Must be able to concentrate with frequent interruptions. Must be able to walk for up to 75% of the workday. Must be able to tour through the entire building, up to one mile, occasionally pushing a wheelchair. Must be able to bend and stoop. Must be able to lift and carry up to fifty (50) pounds independently.
