Key Responsibilities

Develop and implement a marketing program for the facility in coordination with the administrator and care team.

  • Information is received and documented on all new resident referrals and necessary data for completion of all phases of assessment and admission are completed.

  • Screening evaluations are performed on each referral, and criteria to determine whether the prospective resident meets criteria for transfer and admission are objectively applied.

  • Verify financial support.

  • Referring and receiving physician communication is coordinated and facilitated.

  • Communication with the referral source, resident, family (responsible party) insurance representative, and the corporate office related to the status of referrals, progress, and discharge plan is maintained.

  • Documentation on all admissions and discharge activity is maintained.

  • An outcome data tracking system is developed and maintained.

  • Acts as a liaison between Facility and hospital Social Workers/Discharge Planners

  • Advises residents and/or families of services available at the facility

  • Provides written information about the services offered

  • When in the facility, takes all phone calls related to admissions

  • Provides tours of the facility to prospective residents and/or families

  • Works on census development through communication with hospital discharge planners, community home health agencies, hospice organizations, Physician's offices etc.

VIEW JOBS 1/20/2019 12:00:00 AM 2019-04-20T00:00 NOTE: This position will not be filled until May 2019 General Function: The Special Programs Coordinator supports Advancement Via Individual Determination (AVID), Career Technical Education (CTE), and Perkins Grant Implementation at all schools within the district that have AVID, CTE, and/or Grant funded CTE and/or uni/multi- disciplinary programs. The Coordinator has responsibility for ensuring the implementation of the AVID College Readiness, CTE and/or Grant funded CTE program components according to the requirements of the designated programmatic methodologies and ensuring a successful implementation of AVID and CTE programs at the school sites., The Special Programs Coordinator shall attend program related district leadership trainings at various locations and shall visit AVID and CTE sites to in order to increase district capacity and to provide support to those implementing AVID, CTE, and uni/multidisciplinary programs. Education/Training/Experience * Minimum: Level II or III teacher license. Evidence of continued professional development. AVID training and experience in implementing AVID components in the classroom, or commitment to obtain AVID training. Strong belief in AVID and the value it brings to students and the teachers who work with them. * Preferred: Range of educational work at various levels. Knowledge/Skills and Abilities: * Knowledge of AVID and CTE philosophies and methodologies * Ability to coach and model AVID, CTE, and other funded program operational strategies for classroom teachers * Ability to facilitate dialog and discussion * Excellent writing skills * Strong organization skills * Self-starter * Excellent communicator * Visionary leader * Manage, report, and apply for grants in support of AVID and CTE * Monitor spending, work with Business Services and site accountants on purchasing, requisitioning, and invoicing * Liaise with state agency and national program representatives * Use data to develop program improvements * Contribute to overall leadership of the Department Essential Job Functions All functions are performed in accordance with established policies, procedures, safety and environmental regulations, facility and computer security policies and procedures. Functions are subject to change from time to time to meet the needs of the organization. Current essential functions include the following: * Maintains consistent contact with AVID, CTE and other program-related teachers and staff. * Conducts observation sessions and provides feedback to teachers on effectiveness of their AVID, CTE, and other program implementation. * Holds regular meetings with AVID teachers and site administrators to discuss strategies for growing AVID usage by teachers in the school buildings. * Conducts training sessions for AVID, CTE and other program teachers. * Creates and leads a district AVID, CTE and other program advisory team. * Organizes/arranges for AVID, CTE and other program PD within NM with other districts and supporters. * Develops and participates in network of AVID, CTE and other program regional supporters. * Meets with institutions of higher education in order to be current on needs and expectations of newly entering students, coordinate student outreach, tutor employment, AVID summer bridge programs and supporting AVID, CTE and other program students in the transition to post-secondary institutions. * Acts as the district's AVID, CTE and other program Director to the AVID, CTE and other program national organizations as needed. * Coordinates the collection of data required by the national AVID, CTE and other program organization. * Arranges for all out-of-state training for district teachers for AVID, CTE and other program certification.
