VIEW JOBS 9/19/2018 12:00:00 AM 2018-12-18T00:00 The Care Coordinator is responsible for the concurrent and retrospective review of the medical record of patients admitted to the Medical Center, applying nationally recognized medical necessity criteria sets (Interqual/Milliman). The Care Coordinator will act as a Team Leader for his/her assigned unit, composed of a social worker and discharge planner. Additionally, the Care Coordinator will interact with the physicians and other disciplines involved in the patients care, including the patient to facilitate the multi-disciplinary approach to the care of the total patient. The Care Coordinator will facilitate the patient care conferences for her assigned units on a frequent basis. The Care Coordinator will identify the appropriate level of care and facilitate the transfers to that level of care, whether within, or outside the facility. The Care Coordinator may perform simple discharge planning and provide documentation in the patients medical record of the arrangements, as a tool of communication between the various disciplines. They are responsible for calling reviews to the payors timely to ensure authorization of days and will document approved/denied days in the financial system for billing purposes.* Job Specifications - Knowledge and Skills Active RN, LPN licensure. GSW or LCSW may qualify if they are recommended by the Director of CM. Knowledge of and ability to apply nationally recognized criteria to ensure medical necessity of admission and continued stay preferred Acute care hospital clinical experience required Knowledge of CMS Conditions of Participation preferred Knowledge of payor rules/regulations preferred.
