An Adherence Navigator contributes to AHF's mission by conducting proactive outreach and maintaining of patient enrolled into public funding. This role provides support to patients and our organization by reviewing their funding source(s) and connecting them with other members of our benefits team as needed.
Our benefits department works together to ensure our patients have a source of funding for their healthcare and medications. We issue coverage based on funding from federal, state, and county grants and contracts. We provide the appropriate screenings to ensure proper eligibility based on our contractual requirements.
The private insurance industry can be quite challenging to understand. Helping our patient's navigate through the system is a vital part of the program. Being a Covered California enrollment worker will allow you to explain and enroll the patient into health plans offered through the exchange allowing the patient the opportunity to have full coverage rather than being on a county funded program.
In addition to enrolling our patients into medical funding, we also ensure that our patients remain adherent to their prescribed medication regime. Viral load suppression is the ultimate goal and to aid in this goal, our Access and Adherence Navigators screen each patient to help identify any medical or social barriers that could prevent this. Collaboration and networking with medical and social resources both internally and externally is key to the success of this program.
