Requires Bachelors Degree in Behavioral Health Field plus 2 years mental health experience with adult population OR Bachelors Degree (unrelated field ) and 4 years mental health experience with adult population.

Environment and Scope of the Position: (For example, number of direct reports, sales revenue, responsibility, etc.)

1. UTILIZATION REVIEW

A. Collect pertinent data

B. Enter client into CCIS system and complete required paperwork

C. Follow CARF and October Road CQI process

D. Coordinate and receive initial and ongoing approval from LME, Valueoptions 3rd party authorities

2. (Person Centered) TREATMENT PLANS

A. Devise treatment plans that reflect current MH/SA treatment planning and pertain to the needs of the individual family and consumer

B. Complete treatment plans in a timely manner.

C. Follow CARF, DHHS, LME and October Road CQI standards

3. DISCHARGE PLANNING

A. Formulate appropriate post-discharge placement plans that include services for consumer and family that maximize potential for success

B. Inform administrative staff of pending/planned discharges.

C. Complete discharge summary and other paperwork

4 TREATMENT TEAM

A. Facilitate regularly scheduled treatment team meetings

B. Provide clinical expertise and interact with multi-disciplinary treatment team

5. TREATMENT PLAN REVIEWS

A. Review treatment plans on a scheduled basis with treatment team

B. Provide written treatment plan reviews for placement in clinical records as assigned

C. Obtain consumer signatures on all necessary forms

6. CONTACT WITH REFERRAL SOURCES

A. Maintain regular contact with referral sources as indicated

B. Update on consumers progress

C. Develop appropriate discharge plans collaborating with referral sources

D. Prepare and conduct concurrent reviews to referral sources

7. DOCUMENTATION

A. Responsible for timely completion of medical record documentation

B. Documentation of all outside contacts as well as therapy sessions will be maintained

8. CQI & TEAMS

A. Attend and participate in one or more CQI teams annually

B. Participate in peer record review process

C. NCTOPPS Data entry

9. STAFF RELATIONSHIPS

A. Productive, effective and professional relationships with all disciplines

B. Keeps supervisor informed

C. Open to feedback and supervision

10. MUST COMPLETE ALL INITIAL & REQUIRED ANNUAL TRAINING

11. CASE MANAGEMENT

A. Provide transportation for clients to access community resources

B. Emergency services duty on rotation that may include commitment procedures, after hour assessments, crisis planning, and hospital diversion

Profile: (Knowledge/Professional/Technical Competencies)

Requires Bachelor Degree in Social Work, Counseling, or Psychology from an accredited college or university and CSAC certification. Prefer Graduate degree within a professional Human Services discipline and LCAS or LCAS-A.

Experience:

Prefer 1-3 years of experience working with individuals with severe and persistent mental illness. Requires QP status according to 10A NCAC 27G.0104;

This is full time position (40 hours per week) with state benefits including retirement, health insurance, vacation leave, sick leave, and paid holidays. This position is assigned to the Alcoholism and Chemical Dependency Program (ACDP) section of the Department of Public Safety (DPS). The position is located at Black Mountain Substance Abuse Treatment Center for Women in Buncombe County, NC. The ACDP section administers and coordinates chemical dependency screening and substance abuse treatment services within the DPS. This position will provide paraprofessional supportive casework services to clients and families in a treatment center facility. Person will conduct initial screenings and arrange logistics for referral services. This position also assists in the provision of basic substance abuse information services, administers medications provided by the medical staff and applies intervention techniques when needed. This position is responsible for setup, cleanliness and maintenance of food service and equipment areas. Loading and unloading of facility supplies as needed. Other duties include: documentation of all incidents and medical emergencies, completion of shift reports, transportation of clients, security of buildings, grounds, and vehicles and thorough understanding of security and emergency procedures of the facility. This position may work 1st, 2nd or 3rd shift hours. Hours include weekends and holidays on a rotating basis (4 days/10 hours); occasional modifications to work schedule may include holdover or schedule changes on short notice in order to provide mission critical operational needs. * Note* A valid Driver's License with no driving restrictions is required due to all Substance Abuse Workers are required to transport clients in departmental vehicles at any time during a 24- hour period. This transporting of clients may require driving in different types of weather, traffic or road condition.
