We are seeking a full time Activities Aide to join our team, primarily during the day shift! At least one evening shift is required per week and one weekend day shift required per month. A flexible schedule is preferred.

The Activities Aide promotes independence of residents through utilization of general recreation resources. Provides activity services and assists with department functions through completion of supportive duties.

Responsibilities:

  • Incorporate leading/evidence-based practice in quality/safety into every day work.

  • Encourage patient and family involvement in processes involving patient safety.

  • Delivers calendars, newsletters, activity books to resident rooms

  • Assists with the transportation of residents.

  • Organizes and leads activities in 1:1 or group setting, taking into account the needs and interests of individual residents; modify activities as needed to maximize resident success.

  • Know and follow all organizational/departmental policies/procedures.

  • Records resident activity participation daily on the electronic computer documentation system.

  • Requests needed supplies in advance to ensure material availability.

  • Contributes to the resident newsletter and other media by taking pictures and providing information as needed.

  • Plans and promotes resident activities in advance to provide quality programs.

  • Assume responsibility for personal development and education.

Qualifications

High School Diploma or equivalent Required. One year of craft oriented experience preferred and one year Long Term Care experience preferred.

Licensure, Certifications, and Clearances:

Nurse Aide Certification Preferred

Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) Act 31 Child Abuse Reporting with renewal

Act 33 with renewal

Act 34 with renewal

Act 73 with renewal

Older Adult Protective Services Act (OAPSA)

We are seeking an Assistant Director of Nursing (Clinician Fellow - ADON) to join our team. This position will work day shift, 8:00 am to 4:30 p.m and will assist the Director of Nursing in leading the facility's nursing team. The Clinician Fellow - ADON is a highly developed role model of professional nursing practice with focused expertise and accountabilities in one or more of the following: expert clinical practice, teaching/development, and/or evidence based practice. This role partners with management in the achievement of a culture of excellence in professional nursing practice and patient care. The Clinician Fellow, under the direction of the Director of Nursing, assists in the completion of employee evaluations, selection and hiring of staff, and in performance management. Responsibilities: * Serves as a leader in clinical nursing practice, identifying patient and staff focused opportunities for improvement and leading the change process. * Identifies, educates and supports the psychological and learning needs of staff during change and transition. * Practices participatory leadership and is an active member of shared leadership at the unit, facility or system level. * Assumes formal leadership responsibilities for one or more of the following: quality improvement, evidence based practice, GN or new hire on-boarding. Assists the Director of Nursing with the peer review process. * Applies nursing knowledge and skills within the framework of Relationship Based Care, using the nursing process to meet the clinical, psychosocial and spiritual needs of the patient and family. * Practices solid verbal and written communication skills, and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and unit practices. * Patient documentation is comprehensive and promotes communication between caregivers. * Demonstrates critical thinking in the analysis of clinical, social, safety, psychological and spiritual issues for the patient care within an episode of care. * Incorporates national professional organization as well as business unit and health system's goals to improve patient safety, quality and satisfaction of the patient experience in daily work. * Demonstrates knowledge of performance improvement tools and techniques. * Continuously improves the quality of care and the work environment of outcomes and the integration of research and best practices into daily work. * Shares learning from improvements with other units and/or spreads across the business unit or system. * Develops and maintains productive working relationships internally and externally by building teams and relationships through mentoring and modeling uplifting and positive communication. * Coaches colleagues on cultural diversity and addresses workplace horizontal violence and impairment. * Demonstrates accountability to self and others for safe work hours, time management and healthy lifestyles. * Demonstrates interest in the development of others and positively impacts the lives of patients/families, peers, and members of the healthcare team through mentoring, education, and knowledge sharing. * Actively participates in the role of preceptor for student nurses, nurse interns, graduate and experienced nurses and assists with the development of new preceptors in their role. * Mentors and role models quest for continuous learning through formal education and the development of others. * Seeks opportunities to share expertise with other members of the healthcare team within and beyond the clinical unit/department. Qualifications Demonstrated competence as a Professional Staff Nurse. The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures. Minimum 2 years experience required, long term care experience preferred. If BSN not at time of hire, must be enrolled in a program within one year of hire/transfer date and completed within three years of enrollment (i.e. public health, informational technology, administration, etc.). MSN or CNL preferred. Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Exemplary clinical judgment with critical thinking, analytical and problem solving abilities required as related to various aspects of patient care.
