Currently hiring for: FT days Activities Assistant position.

Job Summary:

The Personal Care Community Life Staff is responsible for coordinating and facilitating all activities that involve persons living in personal care. They are also responsible for the administration and customer service of the community.

ESSENTIAL SKILLS/EXPECTATIONS

  • Resident Service- Practices a resident-centered focus. Creating an environment of compassion by acknowledging and respecting the individuality of each resident.

  • Accountability- Takes responsibility for own actions and monitors personal performance to ensure quality resident care and quality services. Follows department's and organization's policies and procedures.

  • Communication- Practices expressing oneself effectively and respectfully. Also listens well, gives and receives feedback, uses constructive criticism and helps to resolve conflicts.

  • Teamwork- Being open to learning from each other and celebrating and creating community together. Cooperates and partners with fellow team members to accomplish goals and shared purposes.

ESSENTIAL FUNCTIONS/PHYSICAL REQUIREMENTS

  • Supports meeting all of the residents' needs by assisting (to the extent of cross training and certification) with tasks to meet identified needs.

  • Meets productivity standards as outlined annually by Community Life Coordinator and Director of Personal Care.

  • Responsible to oversee admission assessments and any other necessary activity documentation. Ensures that activity documentation is being completed in a timely manner and with accuracy for assigned unit.

  • Plan and provide activity programming for all residents in the community. Responsible to complete 1:1 visits with residents as assigned and document accordingly.

  • Utilizes Support Plans to validate and ensure each resident's plan of care is being followed by all members of the team (interview, observation, audits).

  • Works with team to communicate with residents and families before plan of care is changed (when possible) or when there is a change in the resident's condition.

  • Creates community through maintaining "home" in the physical environment enhancing relationships in the community through shared activities of living.

  • Shares ideas with others, deals with workplace issues quickly and professionally; asks for mediation when needed.

  • Facilitates quality improvement programs and ensures Department of Human Services (DHS) regulations are being followed. Sets goals, evaluates progress, and achieves deadlines and measurable outcomes with the team.

  • In conjunction with the Community Life Coordinator, Director of Personal Care and LPN Coordinator, assists with the identification, investigation and resolution of customer service issues.

  • Maintains a safe, comfortable and functional environment by assessing the community for potential hazards. Implement practices utilizing appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness and emergency care.

  • Attends or ensures the Community Life Team is represented on all appropriate councils and teams of the community.

  • Assist on a limited basis with committees working on larger organization-wide events.

  • Willing to participate in staff development and training programs.

EDUCATION/EXPERIENCE/JOB RELATIONSHIPS

  • A high school diploma or GED.

  • Two to four years related experience, memory support/dementia preferred in a Personal Care or retirement facility.

  • Equivalent combination of education and experience.

  • Encouraged to complete Dining Assist program.

  • Encouraged to complete Dementia Training.

  • Encouraged to complete DHS Direct Care Training Course.

  • Basic computer skills needed. Proficient with Microsoft Office, including Word and Outlook.

  • The Personal Care Community Life Staff is responsible to report to the Community Life Coordinator.
