Access Navigator Supervisor 1

Join our team of competent and compassionate professionals as a shift Access Navigator Supervisor in the CBHC at Behavioral Health Network!

Your experience, knowledge and great skills will support the Access Navigation program with the following:

• Providing supervision, training, guidance, support and encouragement to ensure that Access Navigators are maximizing enrollments into care management programs.

• Assisting Access Navigators in implementing effective strategies for increasing both engagement and enrollment in care management programs.

• Monitoring case load size of Access Navigators to ensure efficiency in engagement and enrollment.

• Monitoring Access Navigators' completion of timely documentation in appropriate EHR's.

• Monitoring Access Navigators' collaboration and referrals with internal and external service providers to meet member needs in a timely and efficient manner.

• Providing supervisory support as well as a consultant for Access Navigators in the absence of the program management.

• Demonstrating willingness and ability to treat challenging and diverse client populations.

• Demonstrating cultural competence in dealing with clients with a variety of different cultural backgrounds.

• Demonstrating working knowledge of community resources, and how to help clients access them.

• Providing effective and positive leadership to supervisees.

• Providing high quality supervision to assigned staff utilizing the Yale supervision model.

• Providing orientation to new staff as needed.

• Utilizing progressive discipline and documents appropriately.

• Having an understanding of various grant reporting metrics, assessments, screening tools, and documentation utilized across the Access Navigation Department.

• Participating in On Call rotation as needed for Access Navigation Department

What are we looking for in an Access Navigator Supervisor?

• Minimum of one year experience working with the above populations.

• Previous supervisory experience (preferred).

• High School diploma with 2-3 years of work experience in related health care field (required).

• Associate's degree (preferred) with 1-2 years of work experience in related behavioral health field.

• Minimum of one-year experience working with the above populations.

• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.

• Must have familiarity with Internet, word processing and emails systems.

• Professional demeanor and phone skills in communicating with members and care team members
