Essential Duties and Responsibilities * Check cafeteria area periodically to make sure all is continually stocked and everything is full/working. * Continually wipes and cleans dining room and other associated areas. * Observes any problems with the quality or quantity of food being served. * Assists customers as needed requested with general information, inquires, etc. * May assist other employees in the food service area to help out as needed * Assists cooks and others as needed. * Coordinates personal duties as required by the overall food service operations. * Maintains a variety of products at certain levels in preparation, serving, or storage areas including service machines. * Restocks salad or soup station during service as is needed. * Stocks storage areas with various products. * Washes objects by hand or with cleaning equipment. * Cleans dishes, utensils, pots, pans etc. and places in assigned storage areas in a an organized and orderly manner. * Cleans, sweeps, wipes down, etc. all general dining, kitchen, or storage areas (in some cases), spike rooms, and equipment as well as all floors. * Removes garbage from assigned areas, sets up or breaks down tables, snack bars, display cases, serving lines etc. * Assists with various periodically scheduled large cleaning projects. * This position is not responsible for the supervision of staff. * Other duties that are pertinent to the department or unit's success also may be assigned. Minimum Requirements * High school diploma or GED equivalent. * Municipality of Anchorage Food Handler's Card or ServSafe Certification contingent upon 30 days of hire. * At least six (6) months of related food service experience. * Contract requires employees to speak, understand, read and write English. Under the direction of the CKPH Administrator/DOSS in coordination with the Assistant Director of Nursing, the Clinical Program Manager is responsible for the leadership and coordination of MHS staff and Activity/Recreation Therapists. Responsible for implementing, monitoring and evaluating programming, milieu management of patients, coordination of cases with clinicians, and inter department process improvement for the overall safety and satisfaction of our patients. Serves as a member of the interdisciplinary treatment team and supports the facility's treatment program and philosophy. Additionally, the Clinical Program Manager: * Assists the Administrator and Assistant Director of Nursing at CKPH with aspects of the hiring process, which may include: selection, orientation, and training. * Supervises CKPH staffs' performance and provides regular coaching and feedback on quantity and quality of work being produced. * Coordinates clinical programming and group schedule between departments. * Assures the treatment plans are being implemented in the milieu with the patients providing feedback to and coordinating with the Lead Clinical Therapist on cases for consistency with treatment interventions. * Assists in representing CKPH in hospital matters and supports hospital goals and objectives. * Audits charts for completion and quality with clinical and MHS documentation. * Assists in creating and overseeing special programs for patients. * Provides Clinical coverage by being available for vacation coverage regarding cases, assessments, case management and/or crisis management as needed. * Maintains group schedule and facilitates group as needed to ensure all groups are covered. * Responsible for identifying training needs in the program, developing training and implementing it where needed. * Coordinates clinical, recreation/activity therapy, and nursing needs within the milieu for quality patient care. * Ensures the implementation, integrity and quality of the program are being carried out daily by the team. * Collaborates with all departments and disciplines to assure the patient and organizational needs are met. * Rounds with the all program staff to assure they are getting what they need to be successful in their positions. * Serves as a member of the leadership team at CKPH * Performs other duties as assigned. Job Requirements EDUCATION & EXPERIENCE: This position requires a Master's degree in Psychology, Social Work, or a related field and two years' experience in clinical services in a mental health setting with experience in supervision or leadership and using the DSM. Additionally, the candidate must be eligible for licensure by the State of Alaska (LCSW, LPA, LPC, or LMFT) at hire or within one (1) year of hire. KNOWLEDGE & SKILLS: Must be able to perform assignments with minimal supervision, work successfully under highly stressful conditions, make sound, independent judgment based on scientific and/or ethical principles, adapt to varying workloads and work assignments on a constant basis, and demonstrate effective comprehensive reading skills, strong communication skills, have excellent assessment skills, and have a working knowledge of computers. MINIMUM REQUIREMENTS OF THE POSITION: Must be willing and able to execute the patient de-escalation methods, both verbal and physical; able to complete new hire requirements such as State of Alaska Background Check and Drug Testing; demonstrate special training, knowledge and skills specific to age groups, as well as job and/or program specific competency within the first three (3) months of training; be certified in CPR and Handle with Care Training within the first month of employment and annually thereafter; complete all required mandatory in-services annually; be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire; PPD skin test required annually or chest x-ray tri-annually thereafter. Must be at least 21 years of age to work in a Residential Treatment Center setting. GENERAL WORKING ENVIRONMENT: Working conditions for all employees are as follows, but are not limited to a psychiatric hospital setting: risk involved in the event of aggressive patient(s); workload may include day, evening, night, weekend, and/or holiday shifts. The Discharge Planner is responsible for providing direct and indirect client care; providing psychotherapeutic services to clients and families; serves as a member of the interdisciplinary team supporting the facility's treatment program and philosophy; serves as the primary liaison between the facility and outside agencies for discharge planning, abuse reporting, and continuum of care functions. The Discharge Planner: * Facilitates timely referrals to alternate levels of care and assists family and/or guardian with completion of applications. * Communicates with referral providers about new referrals and ensures that the facility has all necessary information to consider a referral; assists with ensuring that all receiving providers have all necessary clinical materials and information. * Documents case management contacts in progress notes, communicates with therapists/treatment team about contact and updates on the status of discharge planning. * Works with involved agencies by arranging and participating in multi-agency meetings as needed to facilitate a smooth discharge. * Works with Utilization Review to establish prior authorization, travel, insurance application, etc. is coordinated for a timely discharge. * Maintains a shared file where information is updated to review status of faxes, referrals, etc; maintains contact with current providers to update them on the status of patient/course of hospitalization and to obtain information from provider to share with treatment team. * Participates in problem-solving regarding abrupt changes in discharge, coordinates and communicates the discharge plan. * Coordinates travel arrangements as directed by the therapists and communicates all information to therapists in a timely manner; contacts family with travel information and provides copies of necessary travel documents to family if directed by the therapist/treatment team. * Coordinates discharge appointments and communicates to therapist and family if directed by therapist/treatment team. * Maintains documentation on all cases; ensuring progress notes on all case management contact/discharge planning efforts are completed no later than 24 hours after the contact; documents discharge appointments in the discharge paperwork. * Participates in community activities as requested by the Director of Social Services to build relationships with community providers. * Serves as an effective team member and works closely with the Utilization Review team and clinical team. * Makes phone contacts with referral services and outpatient providers weekly. * Performs other duties as assigned.
