Responsible for assessment, planning, coordination, implementation and evaluation of injured/disabled individuals involved in the medical case management process. Working as an intermediary between carriers, attorneys, medical care providers, employers and employees, you will closely monitor the progress of the injured worker and report results back to the employer and insurance carrier. This will ensure appropriate and cost-effective healthcare services leading to a medically rehabilitated individual who is ready to return to an optimal level of work and functioning. Responsibilities Main responsibilities will include but are not limited to: * Using clinical/nursing skills to help coordinate the individual's treatment program while maximizing cost containment. * Serving as an intermediary to interpret and educate the individual on his/her disability, and the treatment plan established by the case manager, physicians, and therapists. Explaining physician's and therapists' instructions, and answering any other questions the claimant may have in an effort to facilitate his/her return to work. * Working with the physicians and therapists to set up medical assessments to develop an overall treatment plan that ensures cost containment while meeting state and other regulator's guidelines. * Researching alternative treatment programs such as pain clinics, home health care, and work hardening. Coordinating all aspects of the individual's enrollment into the programs, and then monitors his/her progress, in an effort to maximize cost containment and minimize time away from work. * Working with employers on modifications to job duties based on medical limitations and the employees functional assessment. Helping employer rewrite a job description, when necessary and possible, in an effort to return the client to the workplace. * May provide testimony on litigated cases. * Coordinates injured workers' appointments and arranges for transportation as needed. * Documenting and reporting all case activity is critical for accurate case management and billing procedures. * Maintaining all case documents in files ensuring a comprehensive and detailed source of information for all parties involved in the case. * Preparing detailed evaluation reports, as per account guidelines, and case recording documenting for each phase of activity as it is completed. * Maintains phone contact with all parties involved to monitor, update, and advance case activity to ensure the progress of the case. * Completing insurance carrier reports on a monthly (or as required) basis, as well as other necessary paperwork for the insurance company, state, or other regulatory bodies. * Maintaining professionalism at all times despite the stressful demands of the position. Capable of maintaining close relationships among all parties involved both in person and over the phone. Must be readily available for and responsive to all parties concerned. * Acquiring and maintaining knowledge of developments in the medical case management field. Keeping abreast of local workers' compensation laws and regulations, as well as other issues related to the case management/managed care industry. This is also critically important in keeping licenses and certifications valid. * Participating in professional associations keeps the case manager informed of events in their field while establishing referral contacts. * May assist in training/orientation of new staff as requested. * Other duties may be assigned. Prior case management experience preferred. * A current, unrestricted license or certification to practice a health or human services discipline in a state or territory of the United States that allows the health professional to independently conduct an assessment as permitted within the scope of practice of the discipline; or * In the case of an individual in a state that does not require licensure or certification, the individual must have a baccalaureate or graduate degree in social work, or another health or human services field that promotes the physical, psychosocial, and/or vocational well-being of the persons being served, that requires: * A degree from an institution that is fully accredited by a nationally recognized educational accreditation organization; * The individual must have completed a supervised field experience, in case management, health, or behavioral health as part of the degree requirements; and * URAC-recognized certification in case management within four (4) years of hire as a case manager * Pursue URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN) upon eligibility. Other state licenses/certifications as required by law. * Valid driver's license required. * Experience in rehabilitation services industry, vocational/occupational/industrial nursing preferred. * Background in state workers' compensation law and practices desirable. * Excellent interpersonal skills, phone manners, and organizational skills. * Ability to set priorities and work independently. * Computer literacy required. R OLE SUMMARY The Clinical Advisor, Special Population, will be a member of the HGB’s Population Health Department which focuses on those beneficiaries with unique care needs. The clinical advisor will offer guidance and support for the beneficiary’s care, specifically focusing case management efforts in the delivery of care to special populations; to include but not limited to Transplants, NCI Clinical Trials, ECHO/EHHC and ICMP/CCTP; providing a comprehensive, holistic approach for care management throughout the continuum of care. The case manager will offer guidance and support of the beneficiary’s care as directed by the patient’s provider(s) and within the scope of the case manager’s licensure. The case manager will assess, plan, coordinate, implement, monitor, and evaluate the medical services required to meet the complex health needs of TRICARE beneficiaries, to maximize each beneficiary’s capacity for self-care, to cost-effectively achieve desired clinical outcomes and to enhance quality of medical care. The case manager will collect and document data to facilitate measurement of case management involvement. As part of the Population Health/ Special Population Team, the case manager, will serve as the primary coordinator and point of contact for the beneficiary for all activities within the medical spectrum, including collaboration with Behavioral Health Case Management on cases with dual diagnosis. They will also coordinate with other members of the Integrated Care Team (ICT) as needed, in addition to MTF UM / CM staff, physicians and providers as necessary and coordinate and arrange services necessary to address the beneficiary’s condition. In their role, the Clinical Advisor, case manager, will collaborate/coordinate services with the ICT or other care management programs, to include, but not limited to, Humana at Home, New Beginnings, and End of Life services until the beneficiary’s needs are met and case closure or graduation is achieved. Performs all duties within the scope of his/her licensure. Our Department of Defense Contract requires U.S. citizenship for this position. Assignment Capsule * Utilize applicable sources of information to identify, assess, and enroll patients requiring case management * Assess, plan, coordinate, implement, monitor, and evaluate the care of each beneficiary under Population Health/Special Population purview across the continuum of care. Develop a cost effective treatment plan that is acceptable to both the beneficiary (patient) and other members of the care team utilizing both evidence based medical information, DoD and community resources. Plan shall include psychosocial issues, home environment and behavioral health needs across the full continuum of care. Summary: The RN Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Administers skilled nursing care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other members of the nursing team. Each staff nurse performs nursing services for residents within a specific area. Essential Duties and Responsibilities: Responsible for coordination of patient care for assigned caseload. Will manage the clinical aspects and oversee the plan of care for patients along with discharge planning. Acts as a resource for other clinicians including Registered Nurses, Licensed Practical Nurses, Home Health Aides, Physical Therapists, etc., who may be working on a particular home care case for which the primary RN is responsible for managing. Monitoring through observations: patient, staff and physician feedback; written and verbal communication; quality improvement/quality assurance activities; records management. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Responsible to communicating with all disciplines involved in the patients care (i.e., Physical Therapy, Occupational Therapy, Speech Therapy, etc.) Initiates and applies preventive and rehabilitative care techniques. Uses a systematic approach to plan, assess, implement, and evaluate consistency between the plan of care and the care plan. Communicates changes in the patients condition to the team leader, physician and the other team members. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment, medication and other necessary items and services are available. Uses equipment and supplies effectively and efficiently. Performs duties in accordance of and under the direction/supervision as defined by the Agencys organizational chart. Requirements: Licenses/Certifications: Current Registered Nurse (RN) licensure in state and CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Active/current agency health requirements. Must complete required department competencies to validate and maintain competency. Education/Experience: Education: Graduate of an accredited school of nursing with Associates or Bachelors degree.
